Please fill out the following form completely. Fundraising accounts can only be applied for and managed online. You must submit proof of federal tax-exempt status. For more information about our documentation requirements, click here.
Applications typically take about 10 business days to be processed after all documentation is received.
Approved accounts are charged a $25 processing fee, which is deducted from your first check request.
Upload federal document declaring non-profit tax ID number and tax-exempt status.
If your organization name and address do not match the name on the IRS letter, submit a letter on letterhead from the organization listed on the IRS letter. The letter must authorize your use of the tax ID number and provide a mailing address.
OBRC recommends all fundraising checks be sent to a nonresidential address or PO Box for account security. If you are unable to provide a nonresidential address, please download, sign, and email this document.
